Leg and Facial Veins

What are spider veins?
Spider veins, also called telangiectasias,
are tiny visible vessels within the skin. They develop most
commonly on the face and legs. They begin as tiny capillaries
and appear as blue or red squiggles. They are small red, blue
or purple veins that commonly appear on the surface of the
thighs, calves and ankles. It is estimated that at least one
third of the adult female population is troubled with this
common problem. People often seek treatment for spider veins
because of cosmetic concern. However, it is also common for
spider veins to cause symptoms, such as aching, heaviness,
itching and night cramps. Spider veins may be isolated or associated
with “feeder” veins. They can also be associated
with larger underlying varicose veins.
What do spider veins look like?
Spider veins usually take on one of three forms: They
may appear in a spider shape with a group of veins radiating
outward from a central point. They may appear in a pattern
that resembles branches on a tree or they may be in a linear
pattern that appears as thin separate lines.
How are varicose veins different?
Spider veins may be isolated or associated with “feeder” veins.
They can also be associated with larger underlying varicose
veins. Varicose veins are distended veins beneath the skin.
They may appear as bluish bumps and often cause discomfort.
Those with unsightly varicose veins and aching pain often choose
to have surgery to remove their varicose veins. Because varicose veins are no
longer functional, their presence is not missed. Ask your general surgeon about
varicose vein treatment. If you have both varicose and spider veins, your varicose
veins must be treated first. Treating spider veins first is generally futile
because, in the presence of varicose veins, they are bound to return soon.
What causes spider veins and varicose veins?
The below factors may play a part in the development of
varicose and spider veins:
• Heredity -
There is a significant relationship between heredity and the
development of varicose veins and
spider veins.
• Age - The development of varicose
veins and spider veins may occur at any age but usually occurs between the ages
of 18 and 35 years, and peaks between 50 and 60 years.
• Gender – Females are
affected approximately four to one to males.
• Pregnancy – Pregnancy
is a common factor contributing to the formation of varicose and spider veins.
The most important factor is circulating hormones that weaken vein walls. There
is also a significant increase in the blood volume during pregnancy. This tends
to distend veins, causing valve dysfunction which leads to blood pooling in the
veins. Additionally, later in pregnancy, the enlarged uterus can compress veins,
causing higher vein pressure leading to dilated veins. Varicose veins that form
during pregnancy may spontaneously improve or even disappear a few months after
delivery.
• Lifestyle/Occupation - People
who are involved with prolonged sitting or standing in their daily activities
have an increased risk of developing varicose veins. Thus, the weight of the
blood continuously pressing against the closed valves causes them to fail, leading
to distention in the veins.
Lifestyle factors that may contribute to the development of varicose
and spider veins are:
• Binding
clothing
• Chronic constipation
• Hormonal changes such as Birth
control pills/estrogen replacement therapy
• High-heeled shoes
• Obesity
• Sedentary lifestyle
• Number of pregnancies
• Westernized population
• Heat
How can I prevent the development of spider veins?
Not all varicose and spider veins can be prevented. But some things can reduce
your chances of getting new varicose and spider veins. These same things can
help ease discomfort from the ones you already have:
• Wear Sunscreen to protect your
skin from the sun and to limit spider veins on the face.
• Exercise regularly to improve
your leg strength, circulation, and vein strength. Walking,
climbing stairs, cycling and swimming keep your calf muscles
in motion to activate the calf muscle pump. This reduces pooling
and pressure in the veins.
• Control your weight to avoid placing
too much pressure on your legs.
• Do not cross your legs when
sitting.
• Elevate your legs when resting as
much as possible- keeping your feet positioned higher than
heart level.
• Do not stand or sit for long periods
of time. If you must stand for a long time, shift your weight
from one leg to the other every few minutes. If you must
sit for long periods of time, stand up and move around or
take a short walk every 30 minutes.
• Wear elastic support stockings
and avoid tight clothing that constricts your waist, groin,
or legs. Support stockings provide external graduated counter-pressure
to aid in venous blood flow to the heart. They reduce pooling
and pressure in the veins. They also may reduce the risk
of forming a deep vein blood clot. Consider wearing them
during long plane or car rides.
• Eat a low-salt diet rich
in high-fiber foods. Eating fiber reduces the chances of
constipation which cancontribute to varicose veins. High
fiber foods include fresh fruits and vegetables and whole
grains, like bran. Eating too much salt can cause you to
retain water or swell.
How can spider veins be treated?
The two options for spider vein treatment are laser and sclerotherapy.
They can be employed independently or in combination. Lana
Gray does not provide sclerotherapy treatment. During your
medical consultation at Lana Gray, we will discuss which veins
will be treated most effectively with laser treatment and which
veins would not be appropriate.
Laser surgery-
New technology in laser treatments can effectively treat spider veins in the
legs. Laser surgery sends very strong bursts of light onto the vein. This
can makes the vein slowly fade and disappear. Lasers are very direct and
accurate. So the proper laser controlled by a skilled doctor will usually
only damage the area being treated. Most skin types and colors can be safely
treated with lasers.
Laser surgery is more appealing to some patients because it does not use needles
or incisions. Still, when the laser hits the skin, the patient feels a heat sensation.
Cooling helps reduce the pain. Laser treatments last for 15 to 20 minutes. Depending
on the severity of the veins, two to five treatments are generally needed to
remove spider veins in the legs. Patients can return to normal activity right
after treatment. For spider veins larger than 3 mm, laser therapy is not very
practical.
Sclerotherapy (larger vein injection)
Sclerotherapy involves injecting a liquid agent through a tiny needle directly
into your spider veins, causing them to contract and collapse. Because this technique
relies upon fitting a tiny needle into a tiny vein, it is best suited to medium
and large spider veins. Afterward, you will wear ace bandages or compression
hose for three to ten days as recommended by your plastic surgeon. You may resume
sedentary activities immediately but should avoid exercise, hot baths, and alcohol
for two to three days. Understand that some spider veins may never be successfully
treated through sclerotherapy or laser therapy.
How can I treat spider veins of the face?
A patient with spider veins in the face will usually choose to have laser therapy
rather than sclerotherapy. Laser treatment may cause minor bruising that usually
improves within a week, but often, recovery is immediate. Two to four treatments
may be required to achieve your desired result. Immediately following treatment,
you may apply makeup for a presentable appearance.
For spider veins of the face or legs- you will continue to see improvement of
the vein appearance for up to three weeks after treatment.
What do I need to do before laser treatment?
For 4 weeks prior to laser treatment, you should avoid sun exposure
as much as possible and use a sunscreen with an SPF of at least 30. Do not use
oils or creams on your skin the day of laser treatment. And any make-up will
need to be removed just before therapy begins. You may continue your normal activity,
medicine and dietary schedule before the treatment. Do NOT take extra Tylenol,
Motrin or aspirin prior to laser treatment. Do let the practitioner performing
the laser treatment know of any changes in your health or medicine (including
vitamins or herbal treatments).
What will happen the day of laser treatment?
A local anesthetic or the application of suction to the surface of the
skin will be used for pain during the procedure. You will not need
the use of local anesthetics requiring needle injection, anesthetic creams or
sedation during this procedure.
Before the procedure begins, you'll be fitted with protective eyewear to shield
your eyes throughout the procedure
What do I need to do after laser treatment of spider veins?
Immediately following treatment, avoid vigorous activity
such as jogging or aerobics for 24 hours. Apply polysporin
to any crusting or scabbing areas. Avoid direct sun exposure
to the affected areas for several weeks. In Florida, direct
sunlight is unavoidable. Please wear sunscreen even if you
only go outside to and from activities.
How will I look after laser treatment?
In most cases, you will look the same with the exception of the treated veins
appearing more pink than blue. Your skin may be pink to lightly red immediately
after the procedure. But; this color change tends to return to normal within
1 day and usually within 1/2 hour. Some patients will experience mild swelling
of the area treated. Again, this is also usually very short lived. You may
want to apply a cold cloth to the area treated immediately after the procedure
very briefly for increased comfort.
On uncommon occasions you may see some crusting or irritation of the areas
treated developing a few days after the treatment.
How many times will I need to have a spider vein treated?
To eliminate the spider veins usually requires
two to three treatment sessions.
In some cases the laser may fail to completely eradicate the spider veins.
How long will the veins stay gone after the laser treatment?
Once a vein is gone, that particular vein should not reappear However,
The laser does
not prevent the development of new spider veins. You will be starting
with a new baseline.
Regardless of treatment, you should anticipate the development of new spider
veins over time, just as you would if your spider veins had not been treated.
Spider vein therapy treats current spider veins but does not prevent new ones.
This is the time to use preventative techniques discussed above and slow new
spider vein development
Are there any side effects to laser therapy?
Possible side effects of laser surgery include:
- Redness or swelling of the skin right after the treatment that disappears
within a few days.
- Discolored skin that will disappear within one to two months.
- Rarely burns and scars result from poorly performed laser surgery.

Pseudofolliculitis barbae
What is pseudofolliculitis barbae?
Pseudofolliculitis barbae (razor bumps) is a common condition
of the beard area occurring in African American men and
other people with curly hair. The problem results when
highly curved hairs grow back into the skin causing inflammation
and a foreign body reaction. Over time, this can cause keloidal scarring which
looks like hard bumps of the beard area and neck
Who gets PFB? (pseudofolliculitis barbae)
PFB is most common on the male face, but it can also happen
on the female and on other parts of the body where hair is
shaved or plucked, especially areas where hair is curly and
the skin is sensitive, such as genital <http://en.wikipedia.org/wiki/Pubic_hair> shaving
(more properly termed pseudofolliculitis pubis or
PFP). Men of African, Mediterranean, Jewish, Nordic, and
other peoples who have naturally coarse or tightly curling
hair will often get 'razor bumps' in their beards
What causes pseudofolliculitis barbae?
After a hair has been shaved, it begins to grow back. Curly hair tends to curl
into the skin instead of straight out the follicle, leading to an inflammation
reaction. PFB can make the skin look itchy and red, and in some cases, it can
even look like pimples. These inflamed papules or pustules can form especially
if the area becomes infected.
If left untreated over time, this can cause keloid <http://en.wikipedia.org/wiki/Keloid> scarring
in the beard area.
How can you treat PFB?
• A 100% effective treatment
is to let the beard grow. Once the hairs get to be a certain length they will
not grow back into the skin. Initially, let the beard grow for 30 days to eliminate
ingrown hairs.
• Washing the beard area with
Lever 2000 soap using a Buf-Puf helps raise the hairs from under the skin and
prevents them from growing back into the skin. This should be done twice a day.
Buf-Pufs are available without a prescription at your local drug store.
• When you resume shaving, use
a single blade razor. Double or triple-edged blade razors cut the hairs too short
allowing them to grow in.
• Alternatively, use electric
hair clippers or a razor with an attachment that leaves the cut hairs long. Aim
to have a 5 o'clock shadow immediately after shaving.
• Shaving every other day, rather
than daily, will improve pseudo-folliculitis barbae. Do
not pull the skin taut when blade and shave in the direction
of the follicle, not against it.
• If you have severe pseudofolliculitis
barbae, ask your doctor whether a prolonged course of tetracycline <http://dermnetnz.org/treatments/tetracycline.html> or
one of its derivatives would be suitable for you.
• Consider laser hair removal <http://dermnetnz.org/procedures/lasers.html> .
In some cases, this is much more effective than any other
measure.
• If you must use a blade, before
shaving wash the face with a mild cleanser such as Cetaphil Lotion, then rinse.
Massage the beard area gently in a circular motion with a warm, moist, soft washcloth
for five minutes. This will free up the hair tips so they can be cut with the
shaver. The warm water will also soften the hairs, making them easier to cut.

Laser resurfacing to improve cosmetic flaws, such as wrinkles, acne scars, and
aging and sun-damaged skin, is the latest scientific breakthrough in skin rejuvenation.
Laser removal of hyperpigmented lesions and complexion
blending has been made safer since the advancement of lasers through more wavelengths and
pulse widths. Prior to the newer lasers, treating
patients of color had a high risk of side effects. One
of the more common side effects of the past was post
inflammatory hyperpigmentation of the skin.
What is the laser?
Laser stands for Light Amplification by the Stimulated Emission of Radiation.
Lasers work by producing an intense beam of bright light that travels in one
direction. The laser beam can gently vaporize and/or ablate skin tissue to improve
wrinkles, scars and blemishes, seal blood vessels or cut skin tissue. The laser
has the unique ability to produce one specific color (wavelength) of light, which
can be varied in its intensity and pulse duration. The newest laser systems have
become remarkably precise and selective, allowing treatment results and safety
levels not previously available.
How does the laser work?
The laser creates a beam of high-intensity light that penetrates deep
into skin tissue where it delivers a controlled amount
of targeted therapeutic heat. Candela’s
patented Dynamic Cooling Device™ (DCD™) technology
protects the upper layers of your skin with a cooling
burst of cryogen. Together, the long-pulse laser and
DCD cooling offer optimal treatment with minimal side
effects.
Using a wand-like laser handpiece, undesired skin cells and wrinkles will be
targeted for change. One of the laser's most significant advantages over traditional
techniques for skin is that laser treatment is relatively bloodless. The procedure
also offers more control in the depth of penetration of the skin's surface, allowing
an increased degree of precision and safety in treating delicate areas. You will
see some immediate results but the appearance will continue to change over the
next 3-5 weeks after laser treatment.
How do I prepare for the treatment?
If you have had a history of perioral herpes prophylactic antiviral
therapy may be prescribed. Your skin should be thoroughly cleaned and dried,
removing any makeup, creams, oils, topical anesthetics or self tanning products
before laser treatment. The intended treatment area should not be exposed to
sunlight or tanning booth light for 14 days prior to treatment.
Based on past history and spot test results, some patients may be treated prior
to and after laser therapy with a bleaching agent, steroid cream or other complimentary
therapies.
It is critical that the laser practitioner
knows all medicine and skin therapy you are using or
have used in the recent past. Some over the counter creams,
prescribed medications or cleaning habits can increase
your risk of side effects.
Does it hurt?
Some clients may feel a slight discomfort with each laser pulse, sometimes
described as the snapping of a rubber band on the skin.
This discomfort resolves in several seconds and is minimized by Candela’s
DCD cooling.
How many treatment sessions will I need?
The number of sessions depends on the treatment you require and the
condition of your skin. Many factors impact the efficacy of individual laser
treatments, including the severity of the condition being treated. Three is the
average number of sessions required for removal of a hyperpigmented lesions.
What can I expect during treatment?
Treatment will take place in your laser technician's office. You will
wear safety eyewear to protect your eyes from the laser
light. During the procedure, the activated laser is carefully passed back and
forth over the skin until the technician reaches the level that will make imperfection
less visible.. Each laser pulse may be preceded and followed by a short pulse
of cooling spray. A treatment could take as little as a few minutes to as long
as an hour, depending on the area and condition being treated. For
certain treatments, we may recommend that the resurfacing be performed in two
or more stages.
What will I look like after the procedure?

What precautions should I take after treatment?
In the months following treatment, it's essential to protect your skin from
the sun. If you must be in the sun, apply a strong
sun block with a SPF of 30 or higher and shade your
face with a visor or wide-brimmed hat. You should also
wear good quality sunglasses that have filters to block
100 percent of UVA and UVB rays. In the first few days
after treatment, don’t scrub or rinse
the treated areas with abrasive skin cleansers. Your
technician may advise additional precautions.
Are there side effects?
Your risks are minimal. Side effects may include redness, swelling,
hyperpigmentation, hypopigmentation, blistering, and
purpura (a laser bruise), in the treated area—these
usually do not last more than a few days. Permanent side
effects are rare. Be sure to discuss potential side effects with your provider.
What should I know about my results?
If you're
like most patients, you will notice a significant improvement in the tone
of your skin. You should be aware that even though your results will be long-lasting,
your face will continue to age. Following your treatment, your natural tendency
towards hyperpigmentation will cause new lesions to form. Using sun protections
and a skin-care regimen will help to maintain your results.

What are Cherry angiomas?
Cherry angioma <http://www.healthscout.com/ency/68/18/main.html> is
a cherry-red to purple colored benign skin tumor of unknown origin that appears
most frequently after age 40. Cherry angiomas are the most common vascular lesions
to appear on human skin. They are made up of clusters of dilated capillaries
on the surface of the skin, which accounts for the cherry-red or purple color.
No one knows exactly what causes them.
Cherry angiomas can occur almost anywhere on the skin, but most commonly on the
torso. They rarely occur on the hands or feet. When they first occur, cherry
angiomas are about the size of a pinhead and do not protrude above the surface
of the skin. However, some grow to 1/4 inch across or more, and become spongy
and dome- or mushroom-shaped.
Can cherry angiomas be prevented?
No. It is unknown why one person develops angiomas and another doesn’t.
We do know that genetics play some role and we frequently find that if one sibling
in the family has multiple angiomas, one of the parents and usually other siblings
will also be affected.
If the cherry angiomas are injured, how do you stop the bleeding?
As with any bleeding site, apply direct pressure to stop the bleeding. Make sure
the site is cleaned properly. If bleeding continues contact your physician for
further care.
Does exposure to the sun intensify the color or size?
There have been some suggestions that increased sun or trauma may increase the
number of lesions, but research has not shown this to be true.
If left alone, will the angioma continue to enlarge?
Most angiomas will remain pinpoint width but they may seem more 3-D or taller
as they age. Some lesions will increase in size as in the picture. They may also
bleed if hit or scraped when shaving. You will continue to get new lesions
in the future with or without treatment. In general cherry angiomas are benign
and treatment is for cosmetic reasons. Lesions that bleed profusely or frequently
will need to be removed also.
Treatment of Cherry Angioma
The most common procedures used are (1) electrosurgery – a minor
surgical procedure during where a special electrical needle-like instrument is
used to remove the angioma, (2) cryotherapy – freezing the affected area
with liquid nitrogen, and (3) laser vaporization – using an intense beam
of light to remove the angioma. This technique involves minimal harm to surrounding
skin tissue. Unless the lesion is particularly large (1/4 inch across or more),
you can expect excellent cosmetic results.
What is the method of removal used at Lana Gray International?
At Lana Gray International, we use the YAG LASER (a wavelength of 1064 nm) to
remove cherry angiomas, small hemangiomas and spider veins. This allows
for rapid removal without scarring or post treatment downtime for the patient.
How many treatments will be needed?
Your first appointment will be a consult with the physician at Lana Gray International
to discuss your needs, concerns and safety. In many cases a ‘spot test‘ will
be performed while you are at the consult appointment. A few of the smaller angiomas
will be gone after the first treatment session. But most patients will require
2-3 treatment visits for complete removal of the lesions present. Laser treatment
will treat existing angiomas but will NOT prevent new angiomas from appearing
in the future.
How often are the treatments?
For the angiomas that need to be treated a 2nd or 3rd time, you will return at
approximately 8 week intervals.
What do I need to do before treatment?
Before treatment with the LASER you will need to stay out of the sun and tanning
booths for 2 weeks. The day of treatment do not use lotion or creams on your
lesions before treatment. If we will be treating angiomas on your
face and you want to wear makeup prior to arrival at Lana Gray International,
you simply need to remove makeup at our office before treatment. If you are
taking any vitamin or herbal supplements or prescription medications, you MUST
discuss this with the physician during your consult and keep us updated on any
changes. Most medications and supplements are safe, BUT some supplements
such as high dose vitamin E will need to be stopped briefly and some prescription
medications are contraindicated when using the laser for vascular lesions
.All specifics will can be discussed with the doctor at your consult appointment.
What do I need to do after my treatment?
Most importantly avoid acute sun exposure, and use spf 30 or greater sunscreen
for the next 4 weeks. For the next 12 hours, avoid strenuous
exercise. But otherwise, you may continue on with your normal daily life. You
may return to work, play with the kids, walk or swim normal distances.
Are there side effects?
Sunburn, sun tanning, booth tanning or prolonged sun exposure without protection
can cause hypo- or hyper pigmentation (lightening or darkening of the skin) at
your treatment sites. You may feel a sting type sensation at the site treated
when treating the forehead, hands or face, however most lesions on the chest,
abdomen, arms and legs are completely painless. Large lesions (larger than 1/3
inch) may leave a temporary dimple when the vessels of the angioma are gone.
This resolves also in 1-2 months.
After removal, will there be a scar?
NO. For the typical angioma treated you will notice a lightening of the lesion
or more commonly a change from red to blue color and then slow disappearance
of the lesion over the next 3 weeks.

What precautions should I take after treatment?
In the months following treatment, it's essential to
protect your skin from the sun. If you must be
in the sun, apply a strong
sun block with a SPF of 30
or higher and shade your face
with a visor or wide-brimmed
hat. You should also wear good
quality sunglasses that have
filters to block 100 percent
of UVA and UVB rays. In the
first few days after treatment,
don’t scrub or rinse
the treated areas with abrasive
skin cleansers. Your technician
may advise additional precautions.
Are there side effects?
Your risks are minimal. Side
effects may include redness,
swelling, hyperpigmentation,
hypopigmentation, blistering,
and purpura (a laser bruise),
in the treated area—these
usually do not last more
than a few days. Permanent
side effects are rare. Be
sure to discuss potential
side effects with your provider.
What should I know about my results?
If you're like most patients,
you will notice a significant
improvement in the tone
and texture of your skin.
Many say that their fresher,
younger-looking complexion
makes them feel more
confident at work and
in social situations.
These results usually
last for about 5 years.
You should be aware that
even though your results
will be long-lasting,
your face will continue
to age. Following your
treatment, your natural
facial movements will
eventually cause any "expression" lines
to recur. Using sun protections
and a skin-care regimen
will help to maintain
your results. However,
some patients decide
to repeat the treatment
after several years to
restore their rejuvenated
look.
How many treatments will be required for a visible change?
For skin tightening, most patients will require 3 treatment
sessions for the area involved. However some improvement
will be seen as early as the first treatment. Touch -up
sessions are generally much faster, requiring only one
pass over the skin.
